Understanding the steps in your journey with Kookaburra Occupational Therapy.
An enquiry form can be submitted by an individual for themselves or a family member, by another health provider or medical professional, or any other support service involved in your life.
Please note: Consent must be provided by an individual or their guardian for the referral to be made.
Your enquiry form will be reviewed by the therapist to determine if we are the best fit service to support you. The more information you can provide at the point of enquiry will make this process easier.
If Kookaburra OT is not the most appropriate provider to meet your needs, we will get in contact with you to let you know and point you in the right direction.
If your enquiry is accepted, you will be added to our enquiry list.
Please note: Due to the limited availability of our service, we do NOT hold a waitlist. Therefore, there are no guarantees that you will be able to access our service in any specified timeframe.
If an available appointment spot becomes available in your area, during your available times, we will contact you to see if you are still interested in the service. You are under no obligation to say yes if you have changed your mind or no longer require the service.
If you engage with another provider, we kindly ask you let us know, so we can remove you from our enquiry list.
We strongly encourage you to refer to other organisations who may have capacity, or who hold a waitlist and are able to guarantee your future service.
When Kookaburra OT is ready to engage with you to commence services, you will be contacted to book in an initial appointment.
At this time, we will send you online forms to complete including our Service Agreement (including quote) and Consent Form. It is vital you return these forms as soon as possible so we can update our records and provide you with a high quality service.
If you have any questions before your initial appointment, do not hesitate to get in contact with us.
Your therapist will meet with you to discuss your current goals and gain background information to assist with our service provision.
It is a good idea to come to this meeting with some ideas about what types of things you would like to gain from working with the occupational therapist, what your goals are, any difficulties or concerns you are having, and what areas of your life give you the most meaning and fulfilment.
You are welcome to have a support person such as a parent, family member, friend, or other support provider present to help you in this first meeting. It can also be a good idea to write some ideas down or take photos to show the therapist what is most important to you.
Based on your goals and needs, the occupational therapist will outline a plan for your therapy. This may include assessments, reports, liaison with other people, development of resources, or therapy sessions.
The occupational therapist will work with you to find a service provision model which suits your goals, needs and preferences.
Once your goals have been achieved, and you and the occupational therapist are satisfied you have no urgent or outstanding concerns, your service provision will conclude.
At this stage, the occupational therapist will talk to you about what future steps you can take. This may include continued practice on your goals at home, referral to external services or supports, provision of reports for the purposes of NDIS reporting, a break in therapy services, or the closure of your referral.
Please note, Kookaburra OT aims to provide goal-directed supports so that we can continue to work with a range of clients. However, we will ensure you are satisfied and confident in your abilities before discharging you from our service.
Our therapists are proud when participants no longer require our input, as it means they can continue living independently in their life.
